As a client of Spa Suite 4, I agree and consent to assessment and treatment. I understand that my personal health information will be collected. I understand that all information that I provide will be kept confidential unless required by law. I understand and consent that my medical information may be shared by the various care providers involved in my care and treatment.


Spa Suite 4 is 100% committed to your satisfaction.

Please read on to ensure you have a GREAT experience at Spa Suite 4.


Cancellation Policy:

If I provide less than 24-hour notice, I will be charged for the full amount of the service I had scheduled. I understand that if I can send someone in my place, I will not be charged.

I understand that I am still responsible for my appointment until I hear back from a staff member confirming they've received my email or phone call requesting a cancellation.

I understand that I will provide a valid credit card to keep on file. I understand the credit card will not be charged for a cancelled service unless I am made aware in advance via email or phone call.

Failure to arrive for your appointment will result in a 100% charge of the reserved service amount.


Arriving Late:

Out of respect and consideration of my time and other clients, please plan to be on time. Arrive 10-minutes early for any appointment in order to get the full time scheduled. If you arrive right on time or late, I can only give you whatever time remains of your appointment and you will be responsible to pay the full length of the session you requested.


*This time is scheduled just for you and only you which is why I require a credit card on file or prepayment to reserve and guarantee your appointment. However, it WILL NOT be charged unless you do not adhere to the Cancellation/No- Show policy.


If you do not wish to save a credit card number on file, no problem! You can prepay your service online with an eGift Card here on my website. Cancellation/No-Show policy will still apply.

As a client of Spa Suite 4, you have agreed to give a minimum of 24 hours cancellation notice. This is industry standard and a basic minimum professional courtesy.


Our business relationship is based on mutual trust and respect which I term a "therapeutic bond". That is why I asked all clients to agree to the policy.


Out of respect for all my client's time, as well as my time, you will be charged for appointments for failure to give adequate notice.
